PAYMENT
+ Registration forms must be accompanied
by payment of the registration
+ Registration fees can be paid by Visa,
Mastercard, American Express and Diners

credit card statement
+ A 4% surcharge will be added to ALL Amercian
Express and Diners Charges
+ Please make cheques payable to Mining
2000 Pty Ltd.
+ Delegates will receive a confirmation
letter and tax invoice
CANCELLATION
1. All cancellations of attendance must be made in writing to Vertical
Events
2. A full refund, less an administration fee will be made on those cancellations
received before Friday 5 October 2012
3. No refunds will be made after 5 October 2012
4. All refunds will be paid after the conclusion of the conference
